The Community Health Services (CHS) SitRep is a monthly NHSE return that reports waiting list position across community services, including the number of patients waiting and the proportion waiting over 18 weeks.

If Yes – complete KPI table
Example: Leg ulcer clinic – 4 week first appointment
Example: Referral to 1st appointment
Example: Seen within 4 weeks
Example: ICB contract
Example: BI/SQPR
Example: Priority pathway where clinically indicated

Section 4 – Adding patients to the Waiting List
Section 5 — Appointment Offers, Cancellations & Active Monitoring
Section 6 — PIFU (Patient‑Initiated Follow‑Up)
Section 7 — DNAs / WNB / No Access
(Example: Following a first DNA, the patient is contacted within 2 working days and offered one further appointment. Where appropriate, cases are clinically reviewed to assess risk and agree next steps. If the patient does not respond or declines to rebook, they are discharged back to the referrer. All actions are documented on the clinical system.)
